(Cambridge, ON) - Member of Parliament, the Honourable Gary Goodyear will host a passport clinic on Saturday, April 25 to assist constituents in applying for their passports. This year's second passport clinic will take place at 1425 Bishop St. N , Unit 3 from 9am to 12pm.

“Based on the turnout and success of our first passport clinic in January, I'm providing a second that will help constituents get their applications ready before the summer", said Goodyear. On June 1, all Canadian citizens will require a passport to enter all ports of entry to the USA.

When applying for a passport, you must submit an original proof of citizenship in the form of a birth certificate or a certificate of citizenship, supporting identification with your current name and signature such as a driver’s license and two identical photographs taken within the last 12 months. You must also remember to have your guarantor sign your passport application, a copy of your supporting identification and one of your photos.

Under the current policy, an eligible guarantor must:
•       Be a Canadian citizen 18 years of age or older;
•       Hold a five-year Canadian passport that is valid or has been expired for no more than one year;
•       Have been 16 years of age or older when they applied for their own passport; and
•       Have known the applicant personally for at least two years.

The Information Sharing Consent Form needs to be signed by all applicants who wish to have their Member of Parliament act on their behalf in exchanging information with Passport Canada regarding their passport application. This is to protect the privacy of individuals with respect to personal information.

Please note that the passport clinic on Saturday, April 25 will not be issuing passports, only helping with the application process and if requested, submitting applications on behalf of constituents. For urgent passport requests, you must go in person to the nearest Passport Canada office, which is located at 40 Weber Street East in Kitchener, ON .
