Minister Gary Goodyear, Member of Parliament for Cambridge-North Dumfries, will host a passport clinic on Saturday June 18th, 2011 at his constituency office from 9:00AM-12:00PM. Staff will be available to help constituents complete and submit their passport applications.
“As students finish up another school year, families are eagerly planning their hard-earned vacations. My annual passport clinics are designed to help families better prepare for the holidays, so parents and children can spend quality time together”, said Minister Goodyear.
Passport applicants must provide original proof of citizenship (e.g. birth certificate or citizenship card), supporting identification with an applicant’s current name and signature (e.g. driver’s license or photo Health Card) and two identical photographs taken within the last 12 months. In order for staff to forward passport applications, applicants must have a guarantor sign a passport application, a copy of supporting identification and one of the photos. Simplified applicants do not need a guarantor. Passports will not be issued during the clinic.
