In order for a Travel to Consent form to be approved, both parents must give consent and sign in the presence of a Member’s staff, who is an  authorized Commissioner of Oath.  Individual(s) given consent to accompany a child(ern) on a trip, must also sign the form in the presence of the authorized Member’s staff.
